La prevalencia de la obesidad se ha ido incrementando en forma alarmante alcanzando proporciones epidémicas a nivel mundial. Según la OMS la obesidad se ha casi triplicado entre 1975 y 2016.

Los orígenes de la obesidad son multifactoriales, tradicionalmente se han identificado como sus principales causas a la actividad física reducida y a la dieta rica en grasas y carbohidratos. Sin embargo, otro factor que contribuye al desarrollo de obesidad es la alteración en la cantidad y/o calidad de sueño. Este último se pone en evidencia cuando se analizan estudios que demuestran que los niños que durmieron menos de lo necesario desde los 3 años tuvieron más probabilidad de padecer sobrepeso a los 7 años; en el caso de los adultos, los que durmieron menos de 5 horas al día presentaron aumento de peso en comparación con los que durmieron 7 horas o más. Otro ejemplo de incremento de peso se observó en trabajadores nocturnos.

Entonces nos preguntamos: ¿durmiendo más o menos podemos inducir el control de nuestro peso?, ¿adelgazar o engordar depende del número de horas de sueño? Describimos la relevancia del dormir como regulador de procesos endocrinos y metabólicos.

 

El ciclo circadiano (q consta de 24hs) regula los tiempos del sistema nervioso para el control de las funciones metabólicas, que varían según el ciclo sueño / vigilia. Esta variación de las funciones metabólicas estaría dada por los cambios en los requerimientos energéticos entre el día y la noche. Durante la vigilia predomina la actividad física, el desgaste energético y también el consumo de alimento y agua, razón por la cual nuestros órganos deben prepararse para el consumo energético, la digestión y utilización de nutrientes. Durante el sueño, por el contrario, se ahorra y se almacena energía, se reducen los procesos digestivos y se llevan a cabo procesos de reparación celular, de descanso y de organización de memorias. Resulta evidente, entonces, que ambos estados, el sueño y la vigilia, son relevantes para el buen funcionamiento del individuo y que implican funciones muy distintas en el organismo; respetar los horarios para que se lleven a cabo estos estados resulta primordial para mantener la salud.

 

La privación de sueño induce una alteración del sistema circadiano, que ocasiona una desregulación del metabolismo con alteración en el ritmo de secreción de hormonas catabólicas / anabólicas, es decir el sistema se vuele ineficiente para dar respuestas adecuadas cuando nos alimentamos, favoreciendo a la obesidad y trastornos metabólicos como diabetes. 

Se observa, también, que las personas con sueño alterado presentan mayor sensación de hambre esto podría estar relacionado con la alteración de las hormonas reguladoras del apetito como la grelina y leptina que actúan en los centros de saciedad.

La leptina, es producida por los adipocitos o células grasas y su liberación a la sangre indica que estos tienen un depósito suficiente de grasa y, por lo tanto, actúa como un inhibidor del hambre. La segunda, la grelina, es producida por el estómago cuando está vacío y su liberación estimula la sensación de hambre.

Pues bien, se ha demostrado que la privación de sueño, es decir, dormir poco, baja los niveles de leptina y aumenta los niveles de grelina.

Resulta importante analizar los patrones de sueño para poder realizar un diagnóstico de las causas de apetito alterado y la propensión a aumentar de peso. De tal manera que la implementación de programas de higiene de sueño y/o detección de trastornos de sueño permita controlar un factor que contribuye a la  obesidad.
